ENROLLMENT REQUIREMENTS:

1. Accomplished Westfields Admission & Health Forms
2. Original copy of latest Report Card(F-138) or authentic copy of academic records with the latest grade ratings.

* If written in another language, an ENGLISH TRANSLATION should be attached. 

3. Letter of recommendation from the previous school Principal or Guidance    Counselor.

4. Copy of birth certificate.

5. Medical Certificate (kindly state if there are any allergies on certain kinds of food or medicines)

6. 3 copies 2 x 2 and 3 copies 1 x 1 photos

7. Foreign applicants should also submit the following:

?   Copy of passport that shows:

      Applicant's date of birth, passport number and signature

      Date of issue and expiry date

      Visa status(length of stay in the Philippines)

?   Copy of Alien Certificate of Registration (ACR) or Immigrant Certificate of Residence(ICR)

?   The Philippine Government requires a SPECIAL STUDY PERMIT for minor foreign students. Exempted from this requirement are children of parents with any of the following visa categories:

      Permanent Foreign Residents

      Aliens with valid working visa

      Personnel from duly accredited International Organizations residing in  the Philippines (ADB, UNDP)

      Special Resident Retiree's Visa (SRRV)

      Special Investor's Resident Visa (SIRV)

      Foreign Diplomatic Personnel
